After injection of medicine the plunger is further advanced to engage with the needle assembly. Once engaged the needle assembly is pulled back inside the barrel.

The plunger projecting out of the barrel is then broken off, and re-inserted in the front end of the barrel. Operation of this device is cumbersome and has risks of needlesticks.
